Somerset Housing First - Complex Needs Housing Pathway (East Area)

The Housing First model is designed to support the most vulnerable individuals within this cohort. It offers a person-centred, holistic approach that prioritises immediate access to secure, long-term housing without preconditions. Through intensive, tailored support, Housing First aims to improve engagement, promote stability, and enable individuals to sustain their tenancies and rebuild their lives. This commissioning initiative seeks to deliver a high-impact Somerset Housing First service in Somerset for individuals with complex needs who have experienced long-term or repeat homelessness. By sourcing and providing stable housing as the foundation for recovery and inclusion, the scheme aims to reduce reliance on crisis services, improve health and wellbeing outcomes, and promote long-term tenancy sustainment.
